The Report of Absence Form for Local 1650, AFO, Local 71, and Exempt employees has been updated
This message comes to you from HFC Payroll:
Effective August 1, the Report of Absence Dynamic Form used by employees categorized as Local 1650, AFO, Local 71, or Exempt has been revised.
The new form will accept absences for one month per form. The top right section of the Report of Absence form has been revised to include a dropdown menu for the month and year, which are now required.
When selecting union affiliation from the dropdown menu, the form automatically provides applicable sections required. For example, union affiliation AFO will only show the Extra Contractual section and the contractual section will no longer appear.
The revised form now has a comment section for employees to enter any comments, including specific hours absent for any partial day.
Conference information is now required when selecting reason code CFN. You will be prompted to include the name and location of conference in the bottom section of the form, in addition to any contractual or extra contractual days missed. No other documentation is required for the Report of Absence regarding conference time.
After you complete the absence form
The Report of Absence is available on the Employee Self-Service portal. Here is a direct link to the updated Report of Absence. [NOTE: Login is required. To find this link, go to Self-Service, select “Employee,” and you will find the Report of Absence form on the next screen.]
After you complete and submit the electronic form, it will be routed to your supervisor, who will receive a link to your form. When your supervisor selects the link, they will have the option to sign the form electronically and submit for processing or return the form to you for revisions.
After the form has been approved, you will receive an email with a link to your completed Report of Absence. The Payroll Department will be automatically notified when your Report of Absence has been approved and submitted and will record the absence. You are always able to review your leave balances using Self-Service.
It is your responsibility to go to the form and fill it out for any month in which you have recordable absences. You will not receive a reminder.
This new form eliminates the need to directly send any further PDF reports to the Payroll Department.
